A new oral medication may soon change how patients manage high cholesterol, one of the leading drivers of heart disease in the United States. Enlicitide, an oral PCSK9 inhibitor, has shown it can dramatically lower LDL (“bad”) cholesterol, offering a potentially more accessible alternative to existing therapies.
LDL cholesterol plays a central role in cardiovascular disease. When levels are too high, these particles accumulate in artery walls in a process called atherosclerosis. Over time, this buildup can restrict blood flow or rupture, leading to heart attacks and strokes. While current treatments like injectable PCSK9 inhibitors are highly effective, they are often underused due to high costs, insurance barriers, and the decreased level of comfort and convenience that come with injections. As a result, roughly 60% of patients nationwide struggle to reach safe cholesterol levels despite available therapies.

In a large phase 3 clinical trial involving 2,909 participants with or at risk for atherosclerotic cardiovascular disease (ASCVD), researchers evaluated how well the drug reduced LDL cholesterol over 24 weeks. About two-thirds of participants received enlicitide, while the remaining group received a placebo. Patients taking enlicitide experienced an average LDL reduction of about 60% compared to the placebo group. In addition to lowering LDL, the drug also improved other important cardiovascular risk markers, including non-HDL cholesterol, apolipoprotein B, and lipoprotein(a). Most importantly, a follow-up study found these improvements remained even after a full year.
As an oral drug, enlicitide may be easier for patients to take consistently and more appealing for physicians to prescribe. Removing the need for injections could reduce hesitation in clinical practice, particularly for patients who are uncomfortable with or unable to access injectable treatments.
By combining strong efficacy with greater convenience, enlicitide represents a promising step forward in cardiovascular prevention. If widely adopted, it could help more patients achieve healthy LDL cholesterol levels and significantly reduce the risk of heart attacks and strokes.
